Gemfibrozil (gemfibrozil,1), chemically known as 2,2-dimethyl-5-(2,5-dimethylphenoxy)pentanoic acid, is a lipid-regulating drug of clofibric acid derivative, which was marketed in the United States in 1982. It can promote peripheral lipolysis, reduce the formation of intrahepatic triglycerides by reducing the uptake of free fatty acids by the liver, and reduce the production of very low density lipoproteins by inhibiting the synthesis of very low density lipoproteins apolipoproteins. It can reduce blood triglycerides and increase blood HDL levels, and mildly reduce LDL cholesterol blood concentrations, but in type IV hyperlipoproteinemia, LDL may be increased. Controlled studies have shown a reduction in the incidence of sudden death and myocardial infarction in severe coronary artery disease. It is indicated for people with severe type IV or V hyperlipoproteinemia in hyperlipidemia, who are at high risk of coronary heart disease and for whom diet control and weight reduction have not been effective. It is also suitable for patients with type IIb hyperlipoproteinemia, high risk of coronary heart disease and ineffective treatment with diet control, weight reduction and other lipid regulating drugs.
